Used by the Course Affiliate Faculty to evaluate the Course
Form each Student needs to fill out and turn in to the Course
Coordinator at the end of the course.
Once you have created your course on the CMS website, entered all the information
required and created your student and faculty rosters you will have only a few forms to
complete and return to the Chapter office.  Below is a list of forms that must be completed
and sent to the Chapter office before you will be allowed to print certificates.
